Clarification Text
This Information Text has been prepared by the Republic of Turkey Ministry of
Health (the "Ministry"), which is the data controller within the scope of Law No. 6698 on the Protection of
Personal Data ("KVK Law"), in order to inform the users of the Central Doctor Appointment System (MHRS) about
their personal data, in accordance with Article 10 titled "Obligation of Data Controller to Inform" of the KVK
Law. The central address of the Ministry, which has the capacity of data controller pursuant to the KVK Law,
is "Bilkent Campus, Üniversiteler Mah. Dumlupınar Bulvarı 6001. Cad. No:9 Çankaya/Ankara 06800".
Identity of the Data Controller
In this system, your personal data listed below can be processed for the
following purposes.
Identity Data:
Your Turkish Republic Identity Number, name, surname, father's name, mother's
name, place of birth, date of birth, age, and gender information, as well as that of your children under the
age of 18 whom you can view under the 'My Authorized Persons' section in MHRS, can be processed for the
purpose of verifying your identity and determining the appointments you can make.
Contact Data:
Your contact information and that of your
children under the age of 18 whom you can view under the 'My Authorized Persons' section in MHRS, can be
processed for the purposes of verifying identity and determining available appointments.
Location Data:
The location information of the place you and your children
under 18 (whom you can view under the 'My Authorized Persons' section) are located is used only instantaneously
and is not recorded, in order for you to easily reach the address of the hospital for which you have taken an
appointment via any navigation tool, according to the preference you set as "always" or "only while using the
app" in the location settings of the MHRS Mobile application downloaded to your smart mobile device. You can
determine when it should be used by choosing your preference. If your location services are turned off, your
location information is not processed by MHRS.
Health Data:
Your and your children under 18's (whom you can view under the 'My Authorized
Persons' section) examination appointment date and time that you have made or cancelled, the
hospital/institution/clinic/Doctor of the appointment, special patient code, patient appointment number,
disability status, vaccination date and time that you have made or cancelled, vaccination appointment number
information can be processed for the purposes of carrying out business continuity activities, conducting
vaccination appointment processes, and executing citizen notification processes (SMS, Push, e-Mail, etc.).
Information regarding whether you have a right to vaccination, which dose appointment you can make, whether you
have the right to receive vaccination at home, and which type of vaccine you have the right to receive is used
instantaneously and is not recorded.
Visual and Audio Data:
Your voice recording personal data
is collected during transactions made via the call center and can be processed for the purpose of ensuring
transaction security for individuals receiving and providing appointment services.
Transaction Security Data:
Records related to login
transactions to MHRS via e-Government or e-Nabız; can be processed for the purposes of ensuring system security,
providing services uninterruptedly, and preventing unauthorized access.
Transfer of Personal Data
Your personal data in the Central
Doctor Appointment System is not shared with third parties in any way, with the reservation of the exemption
cases stated in the first paragraph of Article 28 of the KVK Law, in accordance with Article 8 of the KVK
Law.
Method and Legal Basis of Personal Data Collection
Your
personal data is obtained entirely through automated means (via this system). The legal basis for the processing
of your personal data is that it is mandatory for the data controller to fulfill its legal obligation in
accordance with paragraph (ç) of Article 5/2 of the KVK Law, and that data processing is mandatory for the
legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms
of the data subject, in accordance with paragraph (f); and that it is processed based on the legal reason of
processing by persons under the obligation of confidentiality or authorized persons or authorized institutions
and organizations for the purposes of protecting public health, preventive medicine, medical diagnosis;
treatment and care services, planning and management of healthcare services and their financing, in accordance
with Article 6/3.
Rights of Data Subjects and Application to the Data Controller
Users of the Central Doctor Appointment System may exercise their rights
regulated in Article 11 of the KVK Law by applying to the Ministry within the framework of Article 13 of the KVK
Law and the provisions of the Communiqué on the Procedures and Principles of Application to the Data
Controller.
Written applications to be made pursuant to Article 13 of the KVK Law should be
submitted to the address "T.C. Sağlık Bakanlığı, Üniversiteler Mahallesi, 6001. Cadde, No:9, Çankaya, Ankara";
applications to be made via Registered Electronic Mail (KEP) should be sent to "sb@hs01.kep.tr".
